Our office is pleased to accept your insurance assignment as soon
as your coverage is verified. To help verify coverage, you will need
to provide your current insurance card. We will then file your claim forms
and assist in every way we can.
Please read and know the benefits and limitations of your policy.
Some policies pay a set fee for services while others pay a percentage
of the fees charged. Dental insurance rarely covers 100 percent of
the fees charged —some policies do not cover every type of dental
procedure.
At each appointment, you will be expected to pay any unmet deductible
and the portion of the fees not covered under your policy. We will
accept the assignment of benefits paid by your insurance company directly.
If you do not provide us with an insurance form, you will need to pay
the total fee for treatment.
